REIMBURSEMENT

HOW DO I GET REIMBURSED?

First, make sure that you contacted us when you first get your Administrative Inquiry.  A Fraternal Order of Police, Arizona Lodge 44 Representative will contact you and walk you through the investigative process.  If you have a criminal case or an issue that you think will result in termination, call the Arizona FOP Law Firm at 602-241-0474.

 

Second, fill out the application for Reimbursement form on the right.

Note: you must be a current member of the Fraternal Order of Police, Lodge 44 for Corrections, FOP Catalina Lodge #62, AND opted to be a member of the CIPB.  You also must not be within the 90 day "COOLING OFF" period to collect any reimbursement.

 

Reimbursement also is restricted by the CIPB conditions & limitations located here.

 

You must provide a copy of your Administrative Action Letter (the one that shows your time off) and a current copy of your Pay Stub that shows the pay deduction.
